Read and approve the Membership Agreement form. Once submitted the form will automatically be sent to ePhlebology for review and approval.
Once you have completed the Membership Information Form, an auto-message is sent notifying us that a new member has signed up. You will be assigned an account #, and your account will be activated within 2 business days.
